Position Summary
The Consultant-Trainer in PHC will provide technical support and build the capacity of local PHC teams participating in the grant program piloting the Model of Rehabilitation at the community level in target hromadas. The role focuses on improving the quality and accessibility of PHC services by integrating rehabilitation within PHC competencies, strengthening clinical and organizational practices, and supporting the delivery of integrated, person-centered care in the target hromadas.

Expected duties and responsibilities
Reporting to the Coordinator of rehabilitation at the community level, the Consultant-trainer in PHC will have the following responsibilities. The employee must be willing and able to work in Ukraine.
Key Responsibilities:
Training Design and Delivery
- Develop training materials and curricula tailored for PHC professionals (family physicians, nurses) in line with national PHC standards and international best practices.
- Deliver training sessions using interactive adult-learning methodologies, incorporating case-based discussions and locally relevant scenarios.
- Ensure that training content addresses both clinical and organizational aspects of PHC service delivery.
Proposed Framework for Training Content:
- Practical application of WHO Basic Package of Interventions for Rehabilitation in PHC
- Identification and assessment of rehabilitation needs (use of “red flag” system, validated tools e.g. TUG, Brief-BESTest, QuickDASH etc.)
- Applying the ICF framework in PHC practice
- Basic intervention strategies supporting self-management and functional independence at home and in the community
- Communication and patient interaction in rehabilitation
Supervision and Mentorship
- Facilitate quarterly online supervision sessions for PHC teams from participating facilities, focusing on case reviews, clinical problem-solving, and practical mentoring.
- Conduct site visits (one per pilot hromada) to provide on-the-job coaching and support implementation of improved clinical and administrative practices.
Clinical Guidance and Tools Development
- Contribute to the development of Standard Operating Procedures (SOPs) for common health conditions associated with rehabilitation needs.
- Support local teams in strengthening patient flow and referral coordination.
Capacity Building and Continuous Learning
- Review and recommend open-access learning resources (e.g., NHSU Academy, Physiopedia, OpenWHO etc.) on rehabilitation relevant to PHC professionals working at the community level.
Minimum Qualifications:
- Professional background as a Family Physician or PHC Nurse with at least 5 years of clinical experience in PHC facilities.
- Proven experience in training, facilitation, and clinical mentorship for PHC teams.
- Strong understanding of PHC organization, service delivery, and patient flow within the Ukrainian health system.
- Excellent communication and interpersonal skills, with the ability to engage effectively with healthcare teams.
- Fluency in Ukrainian; working proficiency in English desirable.
